Information for Prospective Parents

• 2011-12 Kindergarten Application (Three - Five Year Olds)     Deadline for lottery Dec. 15th
• 2011-12 Mothers Day Out application (One and Two Year Olds)
• DHS Licensing Requirements for Child Care Centers

Currently Enrolled Children

The Forms listed below are mandatory for currently enrolled children and are due by July 16th.

• Pediatrician Form (forms available at Dr.s office)
• Enrollment/Health Record (3 pages and signed)
• Emergency Security Form
• Flu Form
