Preschool Application for 2012 -2013 turning three (oct, nov, dec) 3s, 4s & 5s
Mother's Day Out Application 2012 -2013 one and two year olds

If your child did not get a spot for 2012/2013 they will be put on a waiting list.
Students on the waiting list, but not yet admitted to our program, must reapply by December 15th 2012 for the following school year.

The Forms listed below are mandatory for currently enrolled children and are due by July 16th.

Pediatrician Form (forms available at Dr.s office)
Enrollment/Health Record (3 pages and signed)
Emergency Security Form
Flu Form
